Comparing hospitals is only part of preparing for life in Barcelona. This guide helps you understand the public and private options, ask the right coverage questions and consider healthcare access when choosing a home.
01 · Understand the system
CAP, CUAP, hospital or private appointment?
Learn the main services before you need them. For urgent problems, use professional guidance rather than trying to decide the right level of care yourself.
CAP
Public primary-care centreYour usual starting point for routine public healthcare. Confirm your registration, assigned centre and appointment arrangements. CAP teams also assess unexpected health problems during their operating hours.
CUAP
Urgent primary-care centreAn option for urgent primary-care needs, including when your usual CAP is closed. Check the appropriate centre and its hours; 061 can advise where to go.
Hospital
Specialist and emergency servicesHospital emergency departments handle more serious cases. Planned specialist appointments are a separate route: confirm the referral or booking arrangements for the service you need.
Private care
Insurer or self-pay arrangementsBefore booking, confirm the doctor, location and treatment with your insurer, or request self-pay prices directly. Ask about authorization and charges rather than assuming a hospital’s name guarantees coverage.

These four Barcelona hospitals are useful starting points for research. Compare the relevant specialist, appointment location, language support and payment arrangements rather than choosing on reputation alone.
Sanitas
Hospital CIMA
On Passeig de Manuel Girona. Its official website provides specialty information, appointments and an international-patient contact service. If you have Sanitas insurance, check your exact policy before booking.

Confirm your arrangements before comparing the price.
Public registration and private insurance involve different checks. Establish what applies to each household member and keep written confirmation of any arrangements you will rely on after moving.
Public healthcare
Check the route that applies to you.
- Confirm eligibility and documents. Ask CatSalut which procedure applies to your residence, employment, pension or family circumstances. Do not assume that holding a NIE completes healthcare registration.
- Arrange your TSI. The individual health card identifies you within Catalonia’s public healthcare system. Follow the official application instructions and confirm your coverage.
- Learn the practical steps. Confirm your assigned CAP, how to book appointments and how to access your records through La Meva Salut. Ask how existing treatment will be continued.
Private insurance
Read the exact policy.
- Scope of cover. Check whether the policy includes the outpatient, diagnostic, hospital and surgical services you need, along with any limits or authorization requirements.
- Start dates and exclusions. Ask about waiting periods and how existing conditions are treated. Answer health questions accurately and request written clarification of any exclusions.
- Total cost and renewal. Compare premiums, copayments and services paid separately. Ask about entry-age rules, renewal terms and how future premiums may change.
Medication deserves a separate check: ask your clinician, pharmacist and insurer how your prescriptions will be supplied and paid for. A consultation being covered does not establish the price of every medicine prescribed.
04 · Plan around your household
Make the arrangements specific to you.
A hospital shortlist cannot tell you how your everyday care will work. Before moving, identify the appointments, support and communication arrangements each household member needs.
Families
Know the children’s care arrangements.
Ask how to arrange pediatric appointments and where to seek help outside normal hours. Bring vaccination and relevant medical records, and discuss any ongoing treatment or support with the receiving team. Confirm arrangements separately for each child.
Ask: “Who should we contact, and what records do you need before the first appointment?”
Retirees
Consider the whole routine.
Look beyond the hospital to regular appointments, pharmacy access and transport. If you expect to need rehabilitation, home assistance or other continuing support, ask who provides it, how it is arranged and what it costs.
Ask: “Could I manage these journeys and arrangements during a difficult week?”
Ongoing treatment
Plan the handover before departure.
Ask your current clinician for a useful care summary, medication list and relevant results. Coordinate the next steps with your receiving provider, including appointment timing and prescription arrangements. Confirm any translation needs before commissioning them.
Ask: “What needs to be arranged now to avoid a gap in my care?”
05 · Healthcare and your address
Test the journey, not just the distance.
Living near a hospital is useful only if it serves your needs. Build the housing search around the care you actually use, alongside shops, friends, transport and the rest of everyday life.
Map your regular destinations.
Identify your likely primary-care centre, pharmacy and any confirmed specialists. Check the exact appointment locations: consultations and tests may take place somewhere other than the main hospital.
Try the complete route.
Compare walking, public transport and taxis at realistic appointment times. Notice hills, crossings, transfers and the walk from the stop to the entrance. Include the return journey and a backup option.
Check access to the home itself.
Look for steps before the lift, heavy entrance doors and awkward internal layouts. Consider how you would manage shopping, a temporary injury or a visitor with limited mobility—not only how the apartment feels on viewing day.
For context: in the US, the same MRI can easily cost $1,000–$3,000 without strong insurance — even more in some cases.
That contrast is one of the reasons many retirees feel significantly more financially secure living in Spain.
